In the early days after he was revived, this was almost universally a very bad sign. Usually meant that he was looking at someone he met as the Winter Soldier, and since he'd been inactive since 1989, usually someone who'd been a child when the Winter Soldier had a run in with their parents. About 30% of the time, that look of recognition meant he was about to get shot at.
But this woman looks a little too young for all that. (Recognizing him, not shooting him, which has no minimum age.) So there might be more going on here. And she does, if he's honest, seem a little familiar.
